What is CLASS, and what does a Case Management Agency do?

CLASS — Community Living Assistance and Support Services — is a Texas Medicaid waiver program that helps people with related conditions live in their own home or family home instead of an institution. A Case Management Agency (CMA) like Ardent Quality Care is your point of contact for coordinating every part of that support.

  • We develop your Person-Directed Plan (PDP) and Individual Plan of Care (IPC) with you and your family.
  • We coordinate with your Managed Care Organization (MCO) and every waiver provider you use.
  • We complete the required visits, reassessments, and documentation on your behalf.
  • We advocate for your rights, preferences, and goals at every step.
